Wirkmechanismus der Fluorochinolone:
Fluorochinolone (Gyrasehemmer) wirken bakterizid durch die Hemmung der bakteriellen DNA-Gyrase.Indikationen für Fluorochinolone:
Gramnegative Erreger inklusive Pseudomonas, Mykoplasmen, Chlamydien, einige Anaerobier; Grampositive Erreger, aber nicht gegen Pneumo- oder Enterokokken.
Harnwegsinfektion (zunehmend Resistenzen), Prostatitis, Pyelonephritis, Epididymitis, perioperative Antibiotikaprophylaxe bei transurethralen Eingriffen, Bronchitis, Pneumonien, Sinusitis, gastrointestinalen Infekten, Weichteilinfektionen.
Pharmakokinetik der Fluorochinolone:
Gute Bioverfügbarkeit bei oraler Gabe, gut gewebegängig, renale Elimination Mombelli u.a. (1999) Naber u.a. (1999).
Nebenwirkungen der Fluorochinolone:
Übelkeit, Erbrechen, Durchfall. Pseudomembranöse Kolitis. Neurotoxisch mit Kopfschmerz, Schwindel, Psychosen bis Krampfanfälle. Allergie, Photosensibilisierung. Knorpelschäden, Sehnenschäden.
Kontraindikationen der Fluorochinolone:
Schwangerschaft, Stillzeit, Kinder (Knorpelschäden), Epilepsie, Schlaganfall, schwere Niereninsuffizienz, QT-Verlängerung.
